IMO a Johanniter cross. I like it!

Exactly, it's a fine, silver breast "Profeßkreuz" of the Johanniter Orden (Maltese Order), to be pinned in place of the usual white cloth cross.

The tiny loops, to securely sew the cross, besides the hinged pin, fitted to the reverse.

An interesting piece indeed.
