Medal Bar with Turkish Red Cross Medals

A very interesting combination of Prussian and Turkish awards to a Medical officer (actually I'm not sure if this would be an officer at all). Does anyone know the difference between the two grades of Turkish Red Cross medals? I'm sure this is quite rare although I think the price is still a bit high.

... Persian writing on the suspension bars is an almost match to the TWM, but ach... such a price!
