It would have originally been one of the 2 varieties of KMSt with round disc-like backing plates. A pity as a $1000+ cross has been significantly de-valued, but the conversion from disc-back to pinback looks old and at least semi-professional to me, so it's probably more like a case of the vet getting sick and tired of trying to attach the unusual KMSt patented method.
A nice obverse though!
