I cannot recall the correct name for No. 4 but it is something Bavarian and red cross/medical related.

It's the Kriegserinnerungszeichen des Bayerischen Landesvereins von Roten Kreuz 1914–1918 [War Commemorative Medal of the Bavarian State Association of the Red Cross 1914-1918], awarded to members of the Bavarian Red Cross for service in the Great War.

This is the ribbon for work in the field and in rear areas of operational zones; the ribbon for meritorious homeland service didn't have the black stripes.
