The majority of these documents never had no photo. I think, and I am not 100% sure on that, it is because of the huge amount of men signed up for the army when the war started and cameras where not all too wide spread. So many soldiers remained without photo. And when they were being awarded orders and medals there was no way of getting a photo made for these documents (too many awardees, often on places in the frontline or directly behind, didn't had the opportunity to be photographed in these conditions). I would love to know the exact reasons so perhaps someone else can jump in here?
From the many Soviet memoires that I read lots of soldiers were supposed to be receiving orders and medals but not enough (or none at all) official documents were available so instead they were given temporarily documents in the field. These often got wet or got lost in battle so without this temporarily document they also never received their order/medal. Many soldiers remained without order/medal because of this.
