Award document...
Chris Lee,
It is an award booklet of an individual.
The rectangle is for where a picture of the individual would go (usually missing from what I've seen) the right page of that is the name of the awardee.
entered into the book, it looks to me to be 2 medals that I am unsure of. Others that read Russian should be easily able to tell you.
I can't vouch for the entries authenticity, could have been entered in 1949 or last week. Others may be able to say more on that.
Here's a link that might help you:

This looks like a Medal Certificate type 2. Two medals are listed:
For Labour Valour
For Distinguished Labour
Name on the certificate is Movsesian(last name) Eraniak(first name) Gabrielovna(middle name)
There should be a stamp Valid Without Photo if no photo is present.
There is another thing. The number is 212675 and this number is too low for type 2 but fits type 1. Type 1 Certificate has a column for an award number next to the award name. This certificate does not have that column. Could you tell me how many ribbons on the State Emblem? I mean, the side ribbons. If there are 11 ribbons then it is type 1, if there are 16 ribbons then it is type 2. I'm just curious.
