USSR release certificate from the military service

Can somebody please help me to know what the last page of this document says? I always have difficulties to read handwritten cyrillic letters.

This is the first time I encounter a "release certificate", so I didn't know what to expect.

No idea at all about the translation , but I suggest you to ask on soviet-awards.com or simply mail Art or Alexei.

Yes, yes, I am here to make it all better.

The last page is his military service record. Your guy was drafted in Tallinn (capital of Estonia) in 1941. He was attached to 921st Rifles regiment, and he was an 82 mm mortars squad commander. The two date entries next are July 1941 and October 1945, looks like start/end dates of service, but I am not sure what "15" next to them means. He was decommissioned according to the Decree of the Supreme Soviet of the USSR of September 25, 1945. Signed by the Head of City military commissariat, a Lieutenant-Colonel.
