Field Medical Cards

Does anyone have any examples of Field Medical Cards and similar documents dating from later than the WW1 era? (Blank or completed - it doesn't matter.) I've only really seen the British WW1 version. I would be interested in other countries' versions as well as later UK versions. Many thanks!
