How do I clean these medals without removing the natural patina ?

My advice is to leave them how they are. Almost anything you use to clean them will remove part of their ageing.

But if you feel you must try and clean them, then a soft tooth brush with a little warm water - make sure you totally dry them afterwards.

I agree. Soft brush and warm water with a little soap is all you can use.
Earswab only on enamel.

I once highlighted the higher parts of a kriegsverdienst medal with a Q-tip soaked in silverpolish. It looked very nice. (If you try it, dont rub the surface too hard, the last thing you want is a contrast too heavy with the patina)
