I think there's a picture of Minister Ley wearing a west wall ribbon & medal in the button-hole in Angolia's FF&F vol 2. Of course, at his level he could wear just about anything and few would have the cahones challenge him on it.
I think the Blood Order ribbon counts as a button-hole ribbon (pocket flap)?

That General Stff tunic is a beaut. The trousers are what really stand out in the set! Thank you for the close-ups.
As far as the lifesaving medal being worn as a button hole ribbon, I am begining to think that Angolia could be wrong as no one has seen or heard of a tunic with it...

I know that you had seen this one before, but I will post it again. I had thought it to be a westwall ribbon until you informed me otherwise.

To my Bender book on the Afrika Korp - the Italo-German ribbon could also be worn from the button hole but I've never seen a pic of this...has anyone else ?

This is the first I have ever heard of this medal being authorized for wear in that manner. I have never seen it worn that way either. If I am not mistaken after the Germans surrendered Africa, there was an order to remove that medal from medal and ribbon bars...
When did Bender say the ribbon was authorized for wear in the button hole? Maybe there was a very short span of time between that authorization and when the medal was disallowed?
