His head looks funny because it is being distorted by the page of the book. Look and you will see the page is bending inward toward the spine of the book.

On the visor, the skull is correctly placed in the upper most position on the band to clear the chin strap..so it probably was there, but a collector pulled it off at his table looking for any stampings on the reverse..
