Looking for 1 document type for a book

For a book chapter encompassing the awards process and the office/personnel pertaining to same I am looking for a clear scan or photo of enough quality to print for 1 document I am still missing.

Realize is a rather rare one, but if anyone has the award document that accompanied an award of the Army Roll of Honor Clasp for any Waffen-SS recipient (any unit) please contact me direct:
