I know Wittstock and Stendal.
Fallschirmschule 1
In Stendal, 1943 in Wittstock/Dosse, wird Fallschirmschule 2; 1943/44 neu als Fallschirm-Schule 1 mit
Feldpost-Nummer in den Niederlanden aus der Fallschirm-Schule 4; wird 1944 Regimentssstab
Fallschirm-Jäger-Ersatz- u. Ausbildungs-Regiment Greve.
Fallschirmschule 2
In Stendal; Winter 1943/44 in die Niederlande; neu aus Fallschirm-Schule 1 in Wittstock/Dosse, Luftgau

1941 in Wittstock; 1942 mit Feldpost-Nummer im Westen; Sommer 1944 Feldpost-Nummer gelöscht.

I am sure Jean-Yves Nasse can give you much more accurate information about the LW jump schools in France and elsewhere but here are some notes I made, which you can add to Tommy's response.
Fallschirmschule I
Established in 1936 at Stendal and moved to Wittstock in December 1942. Fallschirmschule 4 given the designation '1' in 1943 and based at Dreux in France until August 1944.
Fallschirmschule 2
Established at Stendal and moved to Braunschweig-Broitzen in 1942. Disbanded in 1943. Reformed in 1943 at Wittstock from Fallschirmschule 1 personnel.
Fallschirmschule 3
Set up at Braunschweig-Broitzen in December 1942. Moved to Kraljevo/Mataruska Banja, Serbia, in December 1943. Also operated briefly at Papà in Hungary. Closed in late summer 1944.
Fallschirmschule 4
Established in 1942/43 at Dreux. 1943/44 redesignated Fallschirmschule 1. Reformed in Freiburg. Transferred in 1944 to Salzwedel, Luftgau III.
This facility might have continued to function as late as December 1944 but I haven't researched this. I am basing this possibility on a couple of references in accounts of the Ardennes and the Battle of the Bulge to German military specialists given jump training for proposed airborne operations in the Ardennes.
There were no 'Heer' jump schools. Jump training was run exclusively by Luftwaffe personnel and instructors. Army and Waffen-SS parachutists were taught by the Luftwaffe, as this photograph of Waffen-SS officers at Fallschirmschule 3 in May 1944 shows.
