    Where was this information recorded?

We know that when a medal or war badge was awarded, it was entered in the soldbuch and wehrpass.
    My question is,
When an individual completed an action or actions, required to qualify for a medal or warbadge ,where were the actions and dates of the action recorded?
For example,an infantry assault badge required participations in 3 seperate infantry attacks.Where were the 3 seperate infantry attacks and participation dates in the attacks recorded?
Or in the case of a minesweeper badge, one criteria was participation in 3 operational sorties.Where was the sorties and participation dates for the sorties recorded?
I always see wehrpass and soldbuch with award entries but no clue as to exactly what the person did to earn the award(s).

Was all this info kept on seperate personal file at Headquarters?

I have only ever seen combat listings clued into Soldbuchs. I used to have an example but have since sold it and deleted the picture....However, I dont know whether or not the battle entries, found in Wehrpasses, could be used in order to determine if award criteria had been met.

I have examples of documents for the award of the such badges that list the dates and places for the 3 actions required, also his name, unit, date of award and authorisation are on these sheets.

These were normally kept with the Wehrstammbuch I believe, this was kept with his home Wehrmeldeamt/Wehrbezirks Kommando.

However, the actuall dates and actions I believe were recorded on Regimental daily orders and entries were taken for that for the relevant award.....

I am off to work now, however will post a few copies later on.............

Thanks for the further information Ian - Just to correct myself, I noticed that one such document is glued in the back of a Flak Wehrpass I own, and although I have not seen this before, I suppose it could be quite common? - A picture can be found in this thread:

Here is the example I mentioned earlier from a soldbuch to a soldier who fought with the 197ID on the Eastern Front.

          Thanks Guys,

Makes sense.I did think that there must of been somewhere where the info was compiled seperately, instead of being a normal wehrpass or soldbuch entry.
It is understandable that the information was kept with the wehrstammbuch back at the reporting office ,which would of course removed the majority of chance for the information to go missing.Unlike the wehrpass and soldbuchs which had a much more direct chance of being captured or lost through enemy action.
I do see combat day sheets pasted in soldbuchs for CCC,s quite often but as for most other awards, all that i see is the actual award entry.

These first three show the type of ducuments submitted in support of an application for the award on an assault badge.

This is the sort of document often forwarded after a man has been wounded...I'm not sure where it went but the Wehrmeldeamt is a good bet. As can be seen the regimental daily orders confirmed the days.

Here is a doc from a large group that I have to an infantry officer who served in the 15th Pz Div (DAK). It was issues by his unit, and discusses accumulated time toward the IAB. He wasn't actually awarded the IAB until 1944 though, while serving w/ another unit in Russia

These two show the process of applying for a day to be counted as a assault/close combat one - in this case for close combat. The top document if from the battalion commander to regimental HQ with details of the days.
The bottom one is from regimental HQ to battalion confirming this.
