I dont think, that a Feldlazarett had 5 coys...
And the Arzt 1 is absolutely uncommon...
There was a Armee-Feldlazarett 508 and the next higher Armeefeldlazarett had the number 522. There was never a 509 !!!
I say: Bad tag !

I am sorry to say I think its bad as well. I have been looking for a real Feldlazarett tag to add to my collection for several years now.
