RAD service on pg.4 of a soldbuch?

Does anyone have an example of how RAD unit entries would be written on the top line in page 4 of a [Heer] soldbuch? (Everything I've been able to find so far just shows a WMA entry.)

Off the top of my head I can never remember seeing a RAD unit entered on the top of page 4!!!!

I have seen Soldbuch issued by RAD units...these units were then incorporated into the Luftwaffe or Heer!

Section A. Zuletzt zuständige Wehrersatzdienststelle: is for his last responsible Military Replacement Office......I would not expect to see a RAD unit here but Wehrmeldeamt or Wehrbezirkskommando!!!

RAD service is shown in the Wehrpaß on pages 8, 9 and 10.
