Soldbuch - Help with last page entries

Really struggling to make out the last page entries for this soldier.
I know up to 04/45 he was in Hospital in Bad Harzburg for around a year with a bad spinal injury (i think...…..)
I can not make out though on the last pages if he was put into another unit to fight on or admitted to another hospital....died etc
Any of you guys able to work out what happened to him in the last month of WW2 please ?

He was in differet hospitals (Lazarett): Suwalki/Sudauen (Poland), Itzehoe/Holstein, Bad Harzburg -

others I can not read.

The stamp from the last hospitalization is from Lazarett Itzehoe - he was in recovery vacation (Genesenden urlaub).

Looks like this poor guy was pretty bashed up ….in hospital over a year but at least he ended it being alive !!

I think what happened at the end is still unclear from the entries shown.
I agree with what Gerdan and Hank deciphered.

A possible sequence of events in April / May 1945 :
As Gerdan mentioned, he got payment from Res.Laz. Itzehoe (northern Germany) until 30.4.45.
From the same hospital he was granted "Genesenden-Urlaub", as Hank mentioned.
This would be from 10.4. - 25.4.45 and he would spend it in Dorfstadt bei Falkenstein a place in Sachsen,
~50km SW of Chemnitz.

I do not know, when this place was captured by the Allies.
It is also unclear what happened to this soldier.
After finishing his vacation, he should have returned either to the hospital or his replacement unit.
Its not clear whether anything of this happened. But there is also no indication, that he was captured.

On p.21 bottom, he received regular payment for May obviously from a Wehrmacht unit, but we do not know which.
And the second entry deciphered by Gerdan, dated 25.5.45 points to the fact that he was
formally and officially dismissed from the Wehrmacht, but again it is not clear from which unit (or at which location).
