On my monitor, the color looks decidedly grey rather than blue so most likely a Heer cap. Add the missing cockade and it's complete.

The large "A" on the label is a surplus stamp "Ausgesondert". Since it appears on the plastic, you might be able to carefully remove it.

Regarding the dark grey colour and the information from the maker label its a Heer sidecap made by company Peküro in the 60ies sold by the "Kleiderkasse" to an Oberstabsarzt Dr. Greilich (Medic in Majors rank). The Kleiderkasse (short KKBw or KKB) was the BW internal supplier for private purchasable stuff .

Luftwaffe officer's hat for lower range ranks. Unless there's a date I don't think it's possible to know for sure when it was made/used. My guess is early 1960s.
